If you are entering the Czech Republic from a Rabies Free Country or Third Country, you will need to have your pet microchipped with a 15 digit ISO compliant microchip, then vaccinated at least 21 days prior to travel. If your pet's microchip is not ISO compliant, then you will need to carry a microchip scanner.
A licensed veterinarian must complete the official EU Annex II veterinary certificate. If you are traveling from the United States or Canada, the certificate must be sent to the USDA or CFIA for endorsement. If you are entering the Czech Republic from a different country, the instructions will advise you of the proper procedure.

If you are entering the Czech Republic from a Country with a High Incidence of Rabies, quarantine can be avoided by having a Blood Titer Test done after vaccination and 3 months prior to travel. You will need to select the option above for additional forms and instructions.
Your Czech Republic pet passport is verified at the time of order.
If you have more than one pet traveling to the Czech Republic, additional forms can be self printed from the Email Attachment option. If ordering additional forms for the Czech Republic with First Class Mail Delivery Option, forms will receive a 30% discount.
