Pet Passport Kazakhstan 

Kazakhstan requires a Veterinary Certificate for entry. Your veterinarian will complete the certificate to be attached to the rabies certificate to be sent to the USDA or CFIA for endorsement depending on whether you are traveling from the United States or Canada. If you are entering Kazakhstan from a different country, the instructions will advise you of the proper procedure.

If you need more than one Kazakhstan form, additional forms can be self printed from the Email Attachment option. If ordering additional forms for Kazakhstan with First Class Mail Delivery Option, forms will receive a 30% discount.
