Traveling with your pet? Keep all of your pet's veterinary and medical information together with their import documents in one place for easy reference. The Pet Passport & Medical Record folder measures 5 ¾ inches by 8 ¾ inches. The cover is done in a heavy dark blue linen finish with silver printing. There are a total of 16 pages plus the cover. 

 CONTENTS 

• The inside front page contains a picture of your pet along with its description as well as the name and contact information of the pet owner. The pet’s microchip number will be included on this page – a microchip is required when traveling to most countries.
• Inside are custom pages to enter the information on your pets travel and for visa stamps from foreign countries.
• There are several pages to record the pet’s medical history, and to record all of its visits to a veterinarian including the contact information for the pet’s veterinarian. You can also keep a detailed record of all of the pet’s vaccinations.
• A separate page is provided to record the results of a Blood Titer Test if your pet has had one performed – recommended when traveling internationally.
• We have included information that will be helpful when you travel internationally, by airplane or by car with your pet.
• Also included in your purchase is a detachable Pet Passport Folder which holds the pet passport forms for your pet when traveling to another country
• We also include a generic airline veterinary certificate for use when your pet is traveling by air.

A certificate is included with your purchase so that you can order any countries pet passport at NO CHARGE to be delivered to you by Email so that you can print out the documents. The certificate is good for one year from the date of purchase of the Pet Passport & Medical Record Book.
