Pet Passport - Blood Titer Test

The document set includes the instructions for your veterinarian to have the blood titer test performed. Also included are the submission form for the Kansas State University and a pet passport folder (First Class Mail option only).

Additional forms can be self printed from the Email Attachment option. If ordering additional forms with First Class Mail Delivery Option, forms will receive a 30% discount.
