Airline Veterinary Certificate
A veterinary health certificate is appropriate for travel on all commercial airlines. THIS FORM CANNOT BE USED TO IMPORT A PET TO AN INTERNATIONAL COUNTRY. If you need a veterinary health certificate to import your pet to a foreign country, click here.
Choose the airline you are traveling or enter "Don't know" if you have not decided.
NOTE: Pet policies will also be included on an airline when specified.
Take this form to your veterinarian to complete. This form must be completed by a licensed veterinarian for the airline to accept it.
The Airline Veterinary Certificate is included as a part of all of our airline and passport packages. So, if you are ordering a pet passport or airline package, there is no need to order this product.
During periods of very cold weather, an acclimation certificate may also be required. On most airlines, pets are not accepted for transport in the cargo section of the airline when the ground temperature is below 45 degrees Fahrenheit (8 degrees Celsius) at any location on the itinerary UNLESS the pet has a veterinarian's statement of low temperature acclimation (Acclimation Certificate). ADD THIS ITEM FOR COLD WEATHER TRAVEL BY SELECTING THE OPTION ABOVE.
If you order using email option, multiple copies can be printed if you traveling with more than one pet.
There are no refunds on pet immigration forms unless there is a problem with the information or forms provided.
