Pet Passport South Korea
South Korea requires a 15 digit ISO compliant pet microchip and a Veterinary Certificate for entry from a rabies-free country.If your pet's microchip is not ISO 11784/11785 compliant, then you can carry your own microchip scanner.
In addition, if you are entering Korea from a rabies-controlled country, then you will need a Blood Titer Test prior to entry. Then the Veterinary Certificate is attached to the rabies certificate and submitted to the USDA for endorsement if the pet is departing from the United States or Canada. If you are transporting your dog or cat from another country, the instructions will advise you accordingly. If you are entering South Korea with more than 5 animals, you will need an import permit. Check the box above if you are entering South Korea with more than 5 pets for information on how to apply for an import permit. Your order will be increased by $1.50 when selecting this option.

If you will transport more than one dog or cat to South Korea, additional forms can be self printed from the Email Attachment option. If ordering additional forms for South Korea with First Class Mail Delivery Option, forms will receive a 15% discount.

I have been doing research for months, emailing the USDA, K-State Rabies lab, asking the vet, and my recruiter; and I still didn't feel like I had clear information because everyone said something slightly different.
