Pet Passport Panama
Panama requires that your pet be vaccinated for rabies at least 30 days prior to travel. Your veterinarian must then complete the proper veterinary certificate in duplicate and attach the rabies certificate. You must then have it endorsed by the State USDA or CFIA Office depending whether you are transporting a dog or cat from the United States or Canada. If you are traveling from a different country, the instructions will advise you of the proper procedure. Included are both English and Spanish versions of the Veterinary Certificate.
The next step is certification by the Panamian Embassy. You can arrange for house quarantine by filing an application at least two weeks prior to travel.

If you will transport more than one pet to Panama, additional forms can be self printed from the Email Attachment option. If ordering additional forms for Panama with First Class Mail Delivery Option, forms will receive a 15% discount.

While my cat and I have yet to leave for Panama, the information I received needed to allow him into the country seems more than complete per my research. My vet recommended Pettravel since she has had continued success with them for the travel needs of other patients. Fingers crossed!

The only other thing that would be helpful is the phone number to call the Panamanian Embassy or address to send the forms too. Otherwise a great form and presentation.
