Pet Passport Airline Veterinary Certificate
A Veterinary certificate is appropriate for travel on all airlines. Choose the airline you are traveling or enter "Don't know" if you would like a generic certificate. PLEASE NOTE: Instructions and pet policies will also be included on each airline when specified. If you need instructions for more than one airline, please Click Here.
The Airline Veterinary Certificate is included as a part of all of our airline and passport forms. So, if you are ordering a pet passport form or an airline form, there is no need to order this product.
During periods of very cold weather, an acclimation certificate may also be required. On most airlines, pets are not accepted for transport in the cargo section of the airline when the ground temperature is below 45 degrees Fahrenheit (8 degrees Celsius) at any location on the itinerary UNLESS the pet has a veterinarian's statement of low temperature acclimation (Acclimation Certificate). ADD THIS ITEM FOR COLD WEATHER TRAVEL BY SELECTING THE OPTION ABOVE.
There are no refunds on pet immigration forms unless there is a problem with the information or forms provided.

I was travelling from India back to Canada, and knew i was bringing back a pup with me. I found the website online, and was so pleased to get my information in 1 day by email. Printed the info. and read the info back to front. The information is up to date, and very helpful. There are details about crate sizes, costs, how to measure to fit the crate, and the airlines health certificate. I would recommend this service to anyone who is travelling, and wanting to adopt a dog or cat. I still have my well read copy, and have already passed on this companys' info to a travel agency here in Toronto. Jerry at pet travel store was prompt in getting me some of the answers I needed as well! thank you for everything...
My ESA was flying with me for the second time with united. She was registered so I did not really needed, in theory, the health certificate. However since the flight was overseas I decided to added to my papers and the passport docs I got. Nobody asked for it but it was completely worth to have it filled up by my vet and fly with it. If only to minimize the stress of flying with your dog overseas and having anything going wrong. Most of the papers are for animals flying in cargo so I am not sure how useful they are. Also I am sure I could have gotten the form from.United airlines directly. Though pet travel store it was easier more convenient and just what I needed.
I bought this because it had glowing reviews for the most part - but when I went through customs in South Korea, they didn't recognize it at all (even though I specifically purchased the United Airlines version). I was able to get through regardless, but it was a bit nervewrecking when I was checking in.
When I was coming back to the States, customs didn't check my cat's paperwork at all, so I can't say how smooth that part would have been.
Delivery was fast (via e-mail) but not sure about the authencity of the certificate, or if all airlines are aware of this form.
The Veterinary Cert appears to be generic, and could have easily been replicated by any word processing program. Unless your vet does not have a form of their own, don't bother buying this & secure it from your Vet's office.
Flew from Germany to Canada. Matter of seconds getting the cat checked in thanks to the airline form. It was a breeze.
Since I am travelling from Egypt and the local vets equivalent airline form looked inadequate, I was relieved to find this online. Not used it yet, we travel in 2 weeks! Fingers crossed all goes ok!
I have been using it in each of my 4 trips so far and the 5th is coming next week!
